Jewish Agency in SA is pleased to process your Aliyah application. In order to
make Aliyah through the Jewish Agency, you must finalize the process through one
of the Jewish Agency Offices in SA.
In order to
open your Aliyah file we ask that you download
and fill out the attached documents:
1. Aliyah
questionnaire
2. Medical form
3. Confidentiality waiver
4. Entry and
Exit form (if you have made many visits to Israel)
5. Ezrach Oleh form in Hebrew
and English (only the Hebrew one needs to be signed)

In addition, so that
your file can be processed, the following documents are required:
1.
Proof of Jewishness (a letter from a Rabbi overseas on the notepaper of an institution
such as a Synagogue or Beit Din, stating clearly that you are Jewish or your parents'
marriage certificate)
2. A full birth certificate
3. Relevant civil documents
